Checkout fields and age requirements
Gender, Date of Birth, and Phone are added automatically when a Qualiphy product is in the cart. Guest checkout is disabled for Qualiphy orders (patient accounts are required for follow-up access).
Age requirements: Rx consultations require 18+, Good Faith Exams require 13+ with a guardian disclaimer.
